Demographics Sheet

Accurate demographic information, including the patient’s name, address, date of birth, contact details, insurance information, and emergency contacts, is essential for proper communication and follow-up care.

How to submit patient information?

Step 01

Collect the necessary documents listed above (discharge summary and demographics).

Step 02

Ensure all forms are complete and legible.

Step 03

Email the documents securely to our HIPAA-compliant platform at

Step 04

You will receive a confirmation email once the documents have been successfully received.

Step 05

Notify the patient that a medical professional will be contacting them within 2 days Of discharge.
